Edge Colocation Access Cards

Use this form if you have a contract for Edge Colocation space
(Contract PNUM = EDGxxxxx)

Frontier provided access cards are required for wholesale customers to enter Frontier’s facilities.

Interested applicants must complete and submit the Edge Colocation Access Card Application with a current photo. If approved, access card credentials will be supplied via mail.

Access Card Deactivation & Return
